Anger at Us Ban on Aids Scientists

The US government came under scathing attack from senior members of the medical establishment yesterday for blocking scientists from attending the International Aids conference which opened in Bangkok.

The biennial conference, with 17,000 delegates, is more political rally than scientific meeting and bears huge significance for those involved in the fight against HIV/Aids.

The US government has sent only a fraction of its usual contingent of scientists, pleading cost – 50 instead of the 236 who attended the last event in Barcelona in 2002.

The Department of Health and Human Services, headed by the health secretary, Tommy Thompson, was yesterday accused of actively preventing certain US scientists and doctors who had a contribution to make from travelling to Bangkok.

Many suspect that behind the action lies a rift between the US and Aids activists who oppose America’s approach to the global pandemic.

Joep Lange, president of the Sweden-based International Aids Society, which organises the conference, said it had been forced to retract papers that had been accepted for conference sessions after the US scientist authors had been refused permission to come. Many meetings, some to train developing world researchers, have had to be cancelled.

“I really think it is shameful that they restricted the US government participation, particularly when you think they are putting so much money into the fight and people in the field who have to do the job are directly prevented from coming here,” said Dr Lange.

Earlier, the editor of the Journal of the American Medical Association (Jama) had also unexpectedly spoken out. Catherine DeAngelis said that Marc Bulterys, the co-author of a Jama paper who worked for the government’s Atlanta-based Centres for Disease Control (CDC), had not been allowed to accept an invitation to fly to Bangkok to talk about it.

“It stymies the ability of scientists to discuss and learn from each other,” said Dr DeAngelis. “It is wrong.”

She pointed out that the trip would have been paid for by the American Medical Association, not the US government. “It is an incredible example of political pettiness. It is anti-intellectual and it is interfering with scientists and the scientific process and means American government-employed scientists are not allowed to be here to share their knowledge,” she said.

Behind the fracas lies the gulf between the US policies on tackling HIV/Aids in the developing world and those of Aids activists who tend to dominate the big international event. Two years ago, Mr Thompson tried to give a speech at the conference in Barcelona but was rendered inaudible by noisy protests. This year the organisers have asked activists to be more civil and allow those with whom they disagree to be heard.

Although the US has put more money into the fight against HIV/Aids than the rest of the world put together, including $15bn (£8.5bn) pledged by President Bush in January last year, activists are unhappy with the way the money is to be spent. Most of it will go to American-instigated programmes in 15 selected countries which stress the so-called ABC philosophy – abstinence, be faithful and condoms “where appropriate”.

Peter Piot, executive director of UNAids, said last week that abstinence, particularly for women in southern Africa, was often not an option.

Randall Tobias, the former head of the pharmaceutical firm Eli Lilly, who runs the president’s plan for Aids relief, heads the US team in Bangkok. Yesterday he said he had “a very large delegation” with him.

The CDC had offered another scientist instead of Dr Bulterys, he said. “It is true that the person who was the author was not part of the delegation but we offered another scientist and they declined,” he said.

The significance of the conference was emphasised at the opening ceremony by Joep Lange, president of the International Aids Society. The Bangkok event is the first in the Aids-hit developing world since the conference in Durban, South Africa, in 2000.

“Durban was a watershed event that catalysed many developments,” he said. Prices of Aids drugs came down, fundraising was stepped up and there are now plans to put millions on treatment.

“Like Durban, Bangkok could be a watershed event,” he said. “The conference is strategically located in Asia, the most populous continent in the world and home to a quarter of all new HIV infections. Asia still has the opportunity to prevent the epidemic from getting completely out of hand.”

Kofi Annan, UN secretary general, called for leadership from all parts of governments, all the way to the top, which has not been seen in all Asian countries, just as African leaders took years to recognise the crisis and speak out against stigma. “Aids is far more than a health crisis. It is a threat to development itself,” he said.

Leadership was one of three priorities he defined. He also called for infrastructure to be scaled up in Aids-hit countries to allow more people to be treated and he called for a better deal for women who are unable to defend themselves against unsafe sex because of poverty, abuse, violence and coercion by older men. “What is needed is the education of girls,” he said.

WHO says Aids drug target unlikely to be hit

One million people in developing countries are now taking Aids drugs, according to figures released yesterday by the World Health Organisation, but it admitted that the target of 3 million by the end of 2005 is unlikely to be reached.

Jim Yong Kim, the director of HIV/Aids at the WHO and the man who pushed for the world to set the “3 by 5″ target, said fixing a difficult goal had driven the treatment strategy forward further and faster than could have otherwise been hoped.

“We are at 1 million. Who would have guessed we would have got to this point by now? We’re not going to make 3 by 5, but the point is that we’re not going to stop,” he said.

The G8 finance ministers had declared that the goal could no longer be restricted to the 3 million people with HIV most urgently in need of drug treatment, but universal access, he said.

It would change the focus, and efforts would increase to get more women and children on the antiretroviral drugs that suppress the virus and keep people alive for longer.

Some activists are likely to say that progress has been too slow and that a much faster roll-out of treatment is needed as the scale of the pandemic increases. The WHO’s progress report, released yesterday, says there has been acceleration.

In sub-Saharan Africa, the worst-hit region, about 500,000 people are on therapy, more than three times the number last June and nearly double the number six months ago.

In Asia, there has been a tripling of the numbers since June to about 155,000 now.

But the real bottlenecks have been the countries with large populations, including South Africa, Nigeria and India. In South Africa there are about 100,000 people on the drugs, mostly in the private sector.

Yet South Africa had 5.3 million people living with HIV at the end of 2003, which is the highest number infected with the virus in any country in the world.

India is thought to have the next highest number, but is responding slowly to the pandemic. Only 7,000 people in the private sector are on treatment, even though Indian companies make cheap copies of Aids drugs.

Nigeria has about 40,000 on treatment and is trying to scale up.

Action Aid, a campaigning group, blamed the failure to meet the 3 by 5 target on a lack of political will.

“This news is an indictment of leaders in rich and poor countries, who have failed to fully back this vital initiative,” said Felicity Daly, its HIV and Aids policy officer. She called on the G8 to provide increased and sustainable funding.

“We are committed to universal access to HIV treatments by 2010,” said Gareth Thomas, the British international development minister. “We will spend £1.5bn in the next three years on Aids, including £150m on orphans.”

Lee Jong-wook, the WHO director general, emphasised the unique and difficult nature of the enterprise. “This is the first time that complex therapy for a chronic condition has been introduced at anything approaching this scale in the developing world,” he said. “The challenges in providing sustainable care in resource-poor settings are enormous, as we expected them to be. But every day demonstrates that this type of care can and must be provided.”

The more access there is to drug treatment, according to Peter Piot, executive director of UNAids, the more people come to be tested for HIV and get counselling, which is fundamental to preventing the spread of the virus.

There is considerable concern about the plight of children, for whom there are no paediatric formulations of the powerful three-drug cocktails that control the virus.

“Globally, access to HIV care and ART [antiretroviral therapy] has not extended widely to children, despite the fact that 50% of HIV-infected children will die before their second birthday in the absence of treatment,” the progress report says. It suggests that 660,000 children in the world need drug treatment this year.

Aids link throws spotlight on Acambis vaccine

A smallpox vaccine produced by Acambis, the biotechnology group based in Cambridge, could offer protection against the Aids virus, US researchers have found.

Although the company was quick to point out that the research is at a very early stage and based on limited testing, the news captured the imagination of the stock market. Acambis shares rose 15p to close at 381.5p even though it will be at least seven years before the smallpox vaccine can be used in the fight against Aids, assuming it receives the requisite approvals.

The company, considered a world leader in vaccine production, said yesterday it was discussing the findings with researchers at Virginia’s George Mason University.

Laboratory tests showed that blood samples from people vaccinated against smallpox were fives times less likely to become infected by the Aids virus.

Even though a different smallpox vaccine was used in the experiment, Acambis’ new Acam-2000 version is likely to be the focus of future studies.

The company has a £260m contract to supply 209m doses of smallpox vaccine to the US government.

The threat of a smallpox attack by bioterrorists prompted Washington to build a stockpile before the vaccine had received regulatory approval.

The company said of the research: “Acambis considers these findings to be very interesting and that they warrant further investigation. Discussions are ongoing with GMU concerning collaborative work to corroborate the data they have produced.”

Professor Ken Alibek, director of the university’s centre for biodefence, said it was not known why the vaccine appeared to boost immunity to the Aids virus. “There is a strange connection between the discontinuation of the smallpox vaccine in Africa and the emergence of the HIV infection.”

Scientists had discussed the possibility of a link but it had not been scientifically tested.

Sam Fazeli, analyst at Nomura, warned that it could take up to seven years before the vaccine received the necessary clearance.

“Even if successful, we do not believe smallpox vaccine sales for this indication are likely until at least 2009-10,” he added.

Obesity Linked to Aids Fears in South Africa

The Hottentot Venus would no doubt have disputed the claim, but an authority on obesity has blamed the rising number of overweight black women in South Africa on Aids. The claim was made yesterday by Tessa van der Merwe of the International Association for the Study of Obesity, who estimated that about one in three black South African women was seriously overweight.

The factors she blamed it on included:

· HIV-Aids, which has created fears among black women that if they are seen to be losing weight it will be assumed they have the disease

· The tribal belief that fatness reflects wealth

· Violent crime, which makes it difficult to get enough out-of-doors exercise

The generously proportioned Saartjie Baartman – also known as the Hottentot Venus – was carted around Europe in the 19th century as a circus freak before dying there. Her remains were found in a French museum and campaigners persuaded former French president François Mitterrand to allow her to return home in 1994. She has become an icon of the feminist community and a symbol of European colonial attitudes towards Africa long before the advent of Aids and present levels of crime.

Ms van der Merwe said all races suffered a high level of weight problems in South Africa.

“Regretfully, there is also a perception that if a black woman is thin, she might have HIV/Aids or that her husband can’t afford to feed her well,” she said.

Ms van der Merwe said regular, high intensity exercise routines were not possible for many. “There is the reality – it simply isn’t safe to walk around,” she said.

Ms van der Merwe, who heads South Africa’s first obesity clinic, said weight was causing health problems such as heart disease, hypertension and diabetes.
